Oral Health Assessment Form

The student’s dentist fills out this form during the first year of school attended in the district, either TK or Kindergarten. It is submitted in the school front office for the site school nurse. It is due by May 31st of the first year of school.

California Pediatric Tuberculosis Risk Assessment

The California pediatric tuberculosis (TB) risk assessment is a public health tool used to identify children who may need TB testing. It involves a questionnaire about a child's exposure to TB and other risk factors, and is used to determine if further testing, such as a TB blood test or skin test, is necessary to screen for TB infection.
