Household Eligibility Forms (Meal & Universal Benefit Applications)

Benefits of Completing a Meal Application

In order to remain compliant with both federal and state regulations for the National School Lunch and Breakfast programs and the California Universal Meal Program, we ask that all families verify their household eligibility. Completing the form can provide your student(s) and family with valuable benefits, such as:

  • SUN Bucks (Summer Grocery Benefits)
    Receive a one-time grocery benefit of $120 for each eligible child to help purchase food during the summer months.
  • Reduced or Free SAT & AP Exam Fees
    Eligible high school students may qualify for reduced or waived SAT and Advanced Placement (AP) exam fees, helping save on college preparation costs.
  • Free Annual School Photos
    Eligible students may receive a free basic annual school photo package.
  • Discounted Internet, Phone & Utility Services
    Eligible families may qualify for discounted home internet, phone, and utility services through participating assistance programs. (Please note that eligibility requirements may vary by program.)

Instructions for Completing Applications

  1. Verify your household size and income. If your income is at or below the scale, please continue to Step 2. If your income is higher, you DO NOT need to fill out an application.

    * Any person who lives in the household is included in the household size (grandparents, parents, students, infants, other adults, or children).
     
  2. Some families may need to complete ONE application, while others may need to complete TWO. Determine if your household has students who attend a school in Box A or Box B. If your household contains students who attend a school in Box A and Box B, please complete both applications. Please be sure to enter your student's name, the school they attend, grade level, and birthdate when completing either application.

Helpful Hints for Completing Two Applications:

  • First, complete the application in Box A:
    • When listing students, only enter students who attend a school in Box A.
    • Any student in your household who attends a school in Box B should be listed under "Household Members" along with all other members of the household.
  • Next, complete the application in Box B:
    • When listing students, only enter students who attend a school in Box B.
    • Any student in your household who attends a school in Box A should be listed under "Household Members" along with all other members of the household.
