Household Eligibility Forms (Meal & Universal Benefit Applications)

In order to remain compliant with both federal and state regulations for the National School Lunch and Breakfast programs and the California Universal Meal Program, we ask that all families verify their household eligibility. Completing the form can provide your student(s) and family with additional benefits, including reduced testing fees, reduced-price or free school photo packages, reduced PG&E and Wifi services, and NEW this year, summer SunBucks cards.

Complete your application before September 2, 2025, for $120 SunBucks EBT cards for each eligible student!

Instructions for Completing Applications

1. Verify your household income vs. household size using the Household Eligibility Chart below.

2. If the combined household income falls at or below the amount indicated for your household size, please complete an application. If your family's combined household income is above the amount indicated for your household size, it is not necessary to complete the application.

2. Completed applications can be emailed to childnutrition@lvjusd.org or dropped off at the LVJUSD District Office (685 E. Jack London Blvd).

3. Paper forms will be sent home with students at the beginning of the school year.
