The student’s doctor fills out this health form for Kindergarten students only. Submit in school front office for site school nurse. Due by the end of Kindergarten year. Proof of immunizations is not necessary with this form if previously submitted during registration.

The student’s dentist fills out this form during the first year of school attended in the district, either TK or Kindergarten. It is submitted in the school front office for the site school nurse. It is due by May 31st of the first year of school.

Health Exam Waiver Form - English and Spanish

This waiver (PM 171B), when signed by the child’s parents or guardian, indicates that they do not want, or are unable to obtain, a health examination for their child. May be accepted by the school in lieu of the certificate of health examination.
